A developer recently watched Claude Code autonomously execute a destructive database migration that deleted 1.9 million rows from a school platform. The post-mortem was honest: “I over-relied on AI.” The data was unrecoverable. The platform was down.
This will happen again. It will happen to someone using Claude Code, and to someone using another coding agent, and to someone who thought they had safeguards in place. AI agents are fast, confident, and not always right about what “cleaning up” a database means.
Here’s the checklist you should run through before you let any AI agent touch a production database — or even a staging environment with real data.
Before You Start Any Agent Database Task
1. Take a full backup, right now, before the session starts
Not yesterday’s nightly backup. A backup taken right now, before you start the session.

Confirm the backup file exists and has a non-zero size before proceeding. This is the step that saves you. The school platform that lost 1.9M rows did not have a recent enough backup.
2. Test your restore procedure before you need it
A backup you’ve never tested is a hypothesis. Run a restore against a dev environment to confirm the process actually works. Do this periodically, not just in crisis mode.
3. Work in transactions — always
Any migration script Claude Code writes should be wrapped in a transaction that you review before committing:

Tell Claude Code explicitly: “Wrap all migrations in transactions. Do not auto-commit. Show me the row counts before and after each destructive operation.”
4. Never run migrations directly on production from an agent session
Use a migration framework (Alembic, Flyway, Django migrations, Liquibase) that creates a version-controlled, reviewable migration file. Let Claude Code write the migration file. You review it. You run it.
The failure mode to avoid: Claude Code has a tool that executes raw SQL or shell commands, and it runs a DROP or mass DELETE in the same session where it’s “helping” with a feature.

5. Set explicit stop hooks for destructive SQL keywords
Claude Code’s CLAUDE.md / project config supports stop hooks. Configure them:

With stop hooks configured, Claude Code will pause and ask for confirmation before executing any command matching these patterns. This is a thin layer but it buys you review time.
6. Use read-only database credentials for exploration
When you’re asking Claude Code to explore schema, debug queries, or analyze data — give it a read-only connection string. Not your admin credentials. A read-only role can’t delete anything.

Create a read-only role if you don’t have one:

Switch to a write-capable role only when you’ve reviewed the migration plan and decided to proceed.
7. Require explicit confirmation before any irreversible operation
In your session prompt or CLAUDE.md:
Before executing any SQL that deletes, drops, truncates, or modifies more than
100 rows, stop and show me:
1. The exact SQL that will run
2. The expected row count affected
3. Whether this operation is reversible
4. Wait for my explicit "CONFIRMED - proceed" before running.
This isn’t foolproof, but it adds a human-in-the-loop checkpoint at the most dangerous moments.
When Something Goes Wrong Anyway
8. Know your database’s point-in-time recovery options
Most managed database services (RDS, Cloud SQL, PlanetScale, Supabase) offer point-in-time recovery. Know where to find this before you need it:
- AWS RDS: Automated backups → Restore to point in time
- Google Cloud SQL: Point-in-time recovery → must be enabled in advance
- Supabase: Point-in-time recovery available on Pro plan
- PlanetScale: Branches for non-destructive schema changes
For self-hosted PostgreSQL: enable WAL archiving and pg_basebackup for PITR capability.
9. If you catch it mid-execution, kill the connection immediately

10. Document what happened and share it
The school platform developer published a public post-mortem. That kind of transparency helps the whole community understand the failure modes. If an agent destroys your data, document it — you’ll help someone else avoid the same mistake.
The Bottom Line
Claude Code is a powerful tool that will execute exactly what it decides to execute, at the speed of an API call. The safeguards above don’t make it impossible for an agent to cause damage — they make it much harder and give you recovery options when it happens anyway.
The 1.9 million rows are gone because the safeguards weren’t in place before the session started. Get them in place first. Take the backup. Test the restore. Then let the agent work.
